SurveyFlow helps you create professional multi-step surveys and feedback forms directly inside Perfex CRM. Build forms with a visual drag-and-drop interface, divide questions into multiple pages, and provide participants with a clean, modern and mobile-friendly form experience.

Send personalized survey invitations to staff, leads, clients, project participants or a manually entered list. Every recipient receives a unique and secure survey link and can complete the form without logging in.

Surveys can also be created independently without selecting a project, client or lead. Every published survey includes a public URL and iframe embed code for use on WordPress and other external websites.

SurveyFlow uses your existing Perfex CRM email configuration and SMS gateway. No additional SMTP account or separate SMS API configuration is required.

Secure Individual Survey Links

SurveyFlow generates a unique, unpredictable 256-bit token for every recipient. Participants do not need a Perfex CRM account and do not need to log in. Each individual link is connected to its recipient, allowing you to see exactly who completed the survey and who did not.

Optional Social Sharing

• Enable or disable social sharing separately for each survey

Simple Workflow

1. Create a survey independently or optionally associate it with a project, client or lead.
2. Add pages and drag questions into the required order.
3. Configure question labels, descriptions, options, media, files and required fields.
4. Preview the customer-facing survey.
5. Publish the survey.
6. Copy the public URL, embed it on a website, or select an audience and delivery channel.
7. Send invitations, monitor responses and send reminders when required.
8. Review result summaries or duplicate the survey for future use.

Installation

1. Upload the module ZIP file from Setup > Modules.
2. Activate the SurveyFlow module.
3. Configure staff permissions if required.
4. Open SurveyFlow from the admin sidebar and create your first survey.

Required database tables are created automatically during module activation. All module data is isolated inside SurveyFlow tables and no Perfex CRM core file is changed.
