Multi-Page Web to Lead Form Module turns long Perfex CRM lead forms into short, friendly multi-step forms. It is fully responsive, submits over AJAX with jQuery validation, and is built on PHP, HTML5/CSS3 and jQuery. The module is easy to install and use.

Multi-Page Web to Lead Form Module has an easy-to-use form builder that upgrades the core Perfex web-to-lead form builder. Add pages, drag fields onto them, show or hide fields and whole pages with conditional logic, and pick one of 7 modern form themes.

7 Form Themes

Every theme uses your form’s own colours, works on phones and tablets, and supports right-to-left languages.

• Elegant – numbered steps joined by a progress line; completed steps show a tick and can be clicked to go back
• Classic – arrow-shaped wizard tabs
• Standard – a large “1 / 4” page counter with a progress bar
• Modern Card – a soft, raised card with a rounded progress bar
• Minimal – no card, a thin progress line and underlined inputs; blends into any website
• Split – a coloured side panel listing every step beside the form; stacks neatly on mobile
• Conversational – large, friendly questions that appear one after another; press Enter to continue

Smooth Animations

• Pages slide in the direction the visitor is moving, and the form resizes smoothly between pages
• A live progress bar and step counter update as the visitor moves through the form
• Fields with missing answers gently shake and show a clear message right under the field
• The submit button shows a loading spinner while sending
• An animated check mark and thank-you message appear when the lead is received
• Animations switch off automatically for visitors who have turned on “reduce motion” on their device

Documentation:

There is no coding skills needed at all to set up this module. It is just a plug and play script and you are good to go. The Quickstart Guide included with this package takes you step by step through activating the module, from creating a new form, styling it and generating the final Multi-Page Web to Lead form.

How It Works

1. Upload and activate the module from Setup → Modules in Perfex CRM.
2. Create a new form and add pages. The first Header field on each page becomes that step’s name in the progress display.
3. Drag fields onto each page and mark the ones that are required.
4. Optionally open the Conditional Logic tab to show or hide fields and pages based on earlier answers.
5. Choose a Form Theme, your colours and the submit button text.
6. Copy the form link or embed code onto your website. New leads arrive in Perfex CRM like any other web-to-lead form.
